Quality and Patient Satisfaction

 

MPMG physicians are recognized for the excellent medical care and service quality they provide for their patients everyday. Our clinical quality and patient satisfaction programs continue to be rated highly and recognized by independent third parties. For the fifth consecutive year, the Integrated Healthcare Association (IHA) recognized MPMG as a top performer among 230 medical groups in California in clinical quality, patient satisfaction and use of information technology in its Statewide Pay-For-Performance Program.

NCQA Physician Organization Certification

 

Mills-Peninsula Medical Group meets the National Committee for Quality Assurance standards for Physician Organization Certification in Credentialing and Re-credentialing. Achievement of NCQA PO Certification demonstrates that MPMG has the systems, process and personnel in place to ensure the quality of care provided by our physicians.  We perform all credentialing functions on behalf of our contracted health plans, including the routine verification of our physicians’ professional qualifications to ensure that they are in accordance with the standards we set for physicians to practice within our network and to provide clinical care to our patients. MPMG has maintained its PO Certification status since June 2002. 

 

The National Committee for Quality Assurance is an independent, non-profit organization that certifies physician organizations, and accredits managed care organizations.

 

Clinical Quality and Care Management Programs

 

MPMG continues to be fully committed to improving the overall health of our patients through the ongoing evaluation, evolution and expansion of our clinical programs. In 2008, MPMG had positive outcomes in the diabetes, heart disease and asthma management programs. Blood glucose and blood lipid control for our patients with diabetes and heart disease remains among the best in the state. Our percentage for preventative screening has continued to increase each year as well.

 

MPMG continues to focus on prevention, stressing childhood immunizations and routine screening for the early detection of breast, cervical and colon cancer. 

 

We continue to assist our physicians with reports and information to support them in providing excellent clinical care to their patients with chronic conditions such as asthma, diabetes, coronary heart disease and heart failure.  In addition, we send out reminders to our patients about screenings and routine exams for management of these chronic conditions.

 

Disease Management Programs

 

Heart Failure

 

MPMG offers a disease management program for patients with heart failure.  The goal of this program is to improve patients' self management skills and quality of life.  Patients utilize an in-home communication device that records data on key clinical symptoms and self-management behaviors, and provides education and support.   Patients who also have diabetes receive self-management education and support for their diabetes.  A nurse care manager checks patients’ progress and is available by phone Monday through Friday to answer questions and provide support.  Please call 650 240-8108 to learn more about this program. 

 

Diabetes

 

MPMG offers a disease management program for patients with diabetes.  We work with patients and their primary care physician to make sure patients receive all recommended tests and that they have the information and resources they need to control diabetes.  We also send reminders when patients are due for tests and periodic mailings with helpful information.  MPMG also offers wellness coaching to its members with diabetes. Coaching is a dynamic process that will help patients accomplish their diabetes self care goals. Complex Care Management Program

 

A qualified team of nurse care managers and care management coordinators are available to assist patients with complex medical needs to obtain an optimal level of wellness and function. In collaboration with your doctor and other members of the healthcare team, MPMG care management staff coordinate, care, services and resources and work with the patient to develop a care plan based on the patient’s goals and needs.   If you would like more information about the program, you may call 650-240-8084.

 

 

Patient Satisfaction

 

Patient satisfaction continues to be a high priority for MPMG. In order to experience continued success in this area, we have put several initiatives into place.  These include measuring and reporting on patient satisfaction among individual primary care physicians and specialists, and continuing to develop and provide educational programs for physicians and office staff to improve communication, access, and the coordination of care.   

 

We are contacting a large number our patients by phone and completing a survey of their satisfaction with our primary care physicians and specialists.  If you receive a call from MTC, our survey vendor, please take the time to complete the survey.  We value your feedback. 

 

The cornerstone of quality medical care is strong and lasting patient – physician relation.  Many of our physicians have been taking part in a 12 hour communication training program. This program is designed to support physicians in making their patient interactions more successful and to focus their time with you and make the most of your office visit time.
