Heart Failure Program
MPMG offers a disease management program for patients with heart failure. The goal of this program is to improve patients' self management skills and quality of life. Patients utilize an in-home communication device that records data on key clinical symptoms and self-management behaviors, and provides education and support. Patients who also have diabetes receive self-management education and support for their diabetes. A nurse care manager checks patients’ progress and is available by phone Monday through Friday to answer questions and provide support. Please call (650)240-8108 to learn more about this program.
